Tobacco-user Surcharge
The health and well-being of our employees is of great value to Renuity. We want to provide an incentive
to those employees who do not use tobacco, as well as cessation opportunities for those wanting to quit.
Through your organization, you have a new way to quit smoking! Participate in the Quit for Life Program, a
digital quit-smoking program offered for FREE.

Starting January 1, 2024, employees who smoke or use tobacco or nicotine products including, but not
limited to cigarettes, cigars, snuff, chewing tobacco, pipes, electronic cigarettes, electronic vaping
devices, etc. regardless of method or frequency of use will incur a monthly surcharge of $50 added to the
medical monthly employee premium.

Voluntary United Health Care Supplemental Plans:

  • Accident: The plan pays cash directly to you when you experience an accident in your body, such as a car accident, fractures, or dislocations on/off the job.
  • Critical Illness: The plan helps protect employees from costly expenses associated with the diagnosis of a covered illness. All benefits are paid directly to the insured and can be used towards any expense.
  • Hospital: The plan that pays cash directly to you when admitted to the Hospital. It can help pay the costs of a hospital stay and related treatment, health plan deductible, and other out-of-pocket costs.

Voluntary New York Life Disability Plan:

  • Voluntary Term Life and AD&D: During this year’s open enrollment employees can purchase additional life insurance up to a guaranteed maximum amount of
    $230,000 without having to answer any medical questions.
  • STD/LTD: Insurance can pay you benefits if you experience a covered illness or injury that keeps you out of work. Payments may come directly to you or someone you designate and can help pay for things like Groceries, The mortgage, Utilities, or Medical bills.
